Patient Care Coordinator
The Patient Care Coordinator (PCC)
Is responsible for providing comprehensive administrative support to a
physician, assisting with the day-to-day services, and contributing to an environment of clinical excellence. The
Patient Care Coordinator advocates for the patients’ health needs. The Patient Care Coordinator helps to
coordinate patients’ treatment plans, guides them to understand the status of their health, and checks in on their
progress. Compassionate, knowledgeable, and collaborative, the patient care coordinator also collaborates with
other members of the care team to look after patients within the healthcare facility, ensuring that the patient
experience meets Premium Health’s standards of CARE: Compassion, Agility, Respect, and Excellence.
Responsibilities
• Documenting patient medical results
• Communicating test results to patients and/or their family members
• Educating patients about medical conditions, medications, and treatment plans
• Notifying managing physician about unusual or unexpected events or problems
• Replying to patient medical inquiries as instructed by the provider
• Refilling medications as instructed by the provider
• Completing medical forms and writing letters for patients
• Assisting providers with tasks as needed
• Maintaining professionalism and treating patients with respect
Qualifications
• Minimum high school diploma or equivalent
• Highly organized with a strong ability to manage different priorities
• Ability to work with others and use available resources to problem solve
• Ability to exercise sound judgment and discretion in fast-changing situations
• Strong customer service skills
• Effective verbal and written communication skills, communicating with diverse populations, including physicians,
employees, patients, and families
• Listening skills, empathy, and compassion
• Effective collaborator and works well with a team of other caretakers
Physical Requirements
• Prolonged periods of sitting at a desk and working on a computer
• May require periods of standing, bending or reaching
• Must be able to lift up to 15 pounds as needed
