Health/Human Service Case Manager I

Within this role as a Health/Human Service Case Manager I at the Blue Hen Center, you help determine initial/continued ongoing eligibility for all needs-based benefits issued. This includes Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children’s Program (CHIP), Subsidized Child Care, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). Other duties include working with TANF and SNAP Employment and Training vendors; and making referrals regarding client assistance with benefits DSS does not provide (Child Support, foreclosure assistance, or food closets). Workers also respond to the diverse needs of vulnerable and low-income residents by sharing information as well as providing timely and accurate benefits to improve basic needs.
